When you have completed your online registration print out the STATPASS and bring it with you to the clinic!

Please Note: You MUST arrive at the clinic before scheduled closing time or you will automatically be placed in line for the next business day.
Please be sure to have picture ID and your insurance card with you for your visit. Payment is due at the time of service including co-pays.
